On the night of 3 November , 1970. Flying Officer Herbert and Pilot Officer Carver were crewing a Mk 20 Canberra Bomber (Aircraft No. A84-231) of No. 2 Squadron RAAF (Phan Rang). The aircraft had completed a bombing mission north of Da Nang and was returning to base. IFF contact was lost at 2022 hrs and nothing further was heard or seen of this aircraft. An aerial search was conducted but was called off after three days.

Both Herbert and Carver are currently listed as missing and for official purposes presumed dead.
